As the new owners of Valley Speedway, it is our intention to provide a safe and secure environment for our drivers and staff. Our insurance carrier requests completion of the form below before a minor is allowed in the pit area or on the infield. Please consider this requirement as safety measure for all patrons of our facility and compliance with the release is mandatory for minors to participate or be allowed in restricted areas of our facility. THIS INCLUDES ALL MINOR CHILDREN ACCOMPANYING PARENTS IN PIT AREA !!

NOTE: THE FORM MUST BE COPIED FRONT/BACK AND PRESENTED TO THE PIT GATE STAFF ON ONE PAGE. WE CAN NOT ACCEPT THIS FORM ON TWO PAGES. BOTH PARENTS OR LEGAL GUARDIANS MUST SIGN FORM IT IS ACCEPTABLE TO SUBMIT TWO SEPARATE SIGNED FORMS IF NECESSARY.
THERE WILL BE A NOTARY AVAILABLE AT THE PIT ENTRANCE FOR YOUR CONVENIENCE. THE ENTIRE FORM MAY BE COMPLETE AND SIGNED ON PAGE ONE, BUT THE SIGNATURES ON PAGE TWO MUST TAKE PLACE IN FRONT OF THE NOTARY PERSONNEL TO BE VALID.
